Abstract
Acute inflammatory processes of female genital organs cause, in a great number of cases, serious problems in menstrual, sexual and reproductive functions and are marked by long and current relapses.
They are accompanied by the following clinical features: increase of microflora resistance to antibiotics, appearance of medical intolerance, that essentially reduces the efficiency of standard methods of treatment. It results in high frequency (up to 76,6 %) of chronic non-specific inflammatory processes in internal genital organs.
There were 37 patients with uterus and appendages inflammatory diseases, under control, who along with traditional therapy were treated with 8 to 10 infusions of 0.9 % ozonized physiological solution with an ozone concentration of 3000 g/L.At the same time, in the control group 15 patients received only traditional antibacterial, sedative, desensibilizing therapy and vitamins.
It was shown that, at the given category of patients, infusions of ozonized solution plus the traditional treatment, positively influenced the biochemical and cytological parameters of blood, albumin and lipid exchange, endogen intoxication and coagulation ability of bloodOn the background of local inflammatory process subsiding the patients had the increase of curtailing time and bleeding, decrease in activity of plasma procoagulants, protrombin index and fibrinogen.
Some parameters of endogen intoxication were simultaneously reduced: the level of lipid peroxidation, malonic dialdehyde, the level of middle peptides, bilirubin, creatinin. Positive influence of ozone therapy on the albumin synthesis of liver function is revealed.
Application of ozone in the complex therapy of inflammatory diseases of female genital organs allows to improve the treatment results, to diminished the frequency of complications and to reduce the time of treatment.

The cerebrovascular disease is one of the main problems in medical practice, because of the high tendency to produce sequels in those who survive, limiting their physical capacities. The purpose of this study was to evaluate the effectiveness of ozone therapy in the ischemic cerebrovascular disease, in acute phase.
One hundred patients, with this diagnosis, that attended the Neurological Institute during a period of one year, were evaluated. Ozone by rectal insufflation was daily administered (at an ozone concentration of 40 mg/L and 200 mL), for a total of 15 sessions.
These patients were evaluated by clinical examination, blood test and their validity through the multidimensional evaluation method, at the beginning and at the end of the treatment. Antiaggregant medication was associated.
The clinical stage improved in 80 % of patients treated with ozone therapy, with a satisfactory increase of their quality of life.

The Food and Drug Administration (FDA) is amending its regulation on the use of ozone-depleting substances (ODSs) in self-pressurized containers to remove the essential-use designations for albuterol used in oral pressurized metered-dose inhalers (MDIs). Under the Clean Air Act, FDA, in consultation with the Environmental Protection Agency (EPA), is required to determine whether an FDA-regulated product that releases an ODS is an essential use of the ODS.
Two albuterol MDIs that do not use an ODS have been marketed for more than 3 years.
FDA has determined that the two non-ODS MDIs will be satisfactory alternatives to albuterol MDIs containing ODSs and is removing the essential-use designation for albuterol MDIs as of December 31, 2008. Albuterol MDIs containing an ODS cannot be marketed after this date.

Abstract
Background
The therapeutic effects of intravenous ozone in the treatment of arterial occlusive disease have been known for some time. Various authors have shown how this treatment improves the transport and release of oxygen to tissues.
Methods:
Among the main mechanisms underlying this result, the most important is increased 2,3 diphosphoglycerate (2,3 DPG). This enzyme results in a diminished affinity of the hemoglobin for oxygen, thereby enabling the latter ’s increased release to the tissues.
Results:
We compared the results in a sample of patients receiving conventional pharmacological therapy and a group of patients receiving ozone therapy according to our usual protocols.
All the patients were treated using a major own-blood transfusion. The dose used for every transfusion was 5000 ng ozone added to 200 cc uncoagulated blood with citrate. Applications were continued twice weekly for a total of 5 sessions.
A total of 50 pharmacologically-treated patients were included at Fontaine’s second stage.
Another group of 50 patients commenced ozone treatment at the same time. Patients receiving pharmacological treatment presented improved symptoms in 50% of cases.
Those who were treated with ozone treatment showed improved symptoms in 86% of cases.
Changes in 2,3 DPG evaluated before and after treatment were correlated with the improved symptoms achieved.
In general low indices of 2,3 DPG corresponded to enhanced levels after treatment and improved symptoms. However, significant improvements were noted even in the event of high enzyme levels before treatment.
Conclusions:
The predictive criterion for cases with low indices of 2,3 DPG at the start of treatment is indicative and not absolute.
Ozone activity induces improvements that exceed all expectations when the patients to be treated present a metabolic reserve requiring stimulation and strengthening.

Rheumatoid arthritis (RA) is a chronic inflammatory disease characterized by invasion of hyperplastic synovial cells and progressive joint destruction. Ozone therapy has been proposed as an immunomodulator and cellular metabolic activator which shows long-term anti-inflammatory effects and serves to reduce further the proinflammatory factors. We purified RA synovial fibroblast cells (RA-SFc) from patients and avoided contaminating macrophages by flow cytometry, then treated them with ozone.
Following the observable decreased production of proinflammatory factors TNF-alpha, IL-1beta, and IL-6 from RA-SFc, we infused the cultured RA-SFc into joints of severe combined immunodeficiency mice. The mRNA and protein levels of the RA-SFc exposed to 3% and 5% ozone were the same.
As a result, 3% and 5% ozone applied externally ameliorated the inflammatory reaction of RA without toxicity or serious side effects.
Therefore, ozone injected into the knees of RA patients could become a valuable treatment, and we confirm the interactive mechanism between ozone and RA-SFc.

Ozone(O(3)) is a significant component of atmospheric air pollution and produces detrimental effects in the lung. Although the mechanism of O(3)-induced lung inflammation and injury is unclear, the increased release of the proinflammatory cytokine tumor necrosis factor-alpha (TNF-alpha) by lung cells following O(3) exposure may shed some light on this subject.
To investigate the role of TNF-alpha in the O(3)-induced pulmonary insult, we intraperitoneally injected rats with either rabbit preimmune serum or rabbit antirat TNF-alpha 1 h prior to O(3) exposure. Approximately 12 h after the end of O(3) exposure the animals were sacrificed, the lungs lavaged, and tissue samples collected for expression of cytokine genes relevant to inflammation.
The bronchoalveolar lavage fluid (BALF) was analyzed for albumin as a marker of pulmonary epithelial permeability changes and for fibronectin for its role in lung injury and repair. The lavage cells were collected, counted, and identified to quantitate the inflammatory response.
Ozone exposure resulted in a significant increase in BALF albumin and fibronectin as compared to air-exposed controls and a significant increase in BALF polymorphonuclear leukocytes (PMNs).
Antibody treatment produced a significant decrease in BALF albumin and PMNs as compared to O(3)-exposed rats given preimmune serum. Antibody treatment did not affect the BALF fibronectin concentration or the total cell count in the BAL.
Tissue analysis for gene arrays revealed an activation of IL-1alpha, IL-6, and IL-10 in animals exposed to O(3). The gene expression was downregulated in animals treated with anti-TNF-alpha antibody prior to O(3) exposure.
The results suggest a central role for TNF-alpha in the mechanistic pathways critical to lung inflammation. The significance of TNF-alpha in the inflammation and epithelial injury produced by ozone exposure reflects its overall contribution through modulation of other cytokines.

The treatment of the acute herniation disk can be approached through various methods, some of them being non-surgical like discolisis with oxygen-ozone therapy (O2-O3). The aim of this paper is to analyse the progressive changes occurring in the RMI and the absence of correlation between the former and the clinical improvement of the symptoms in patients undergoing ozone therapy due to acute disk herniation with root compression.
Between April 2002 and December 2003, 122 patients with herniation disc pathology underwent oxygen-ozone therapy discolysis. Fourty of them with acute herniation lumbar disk, of whom 28 (70 %) were male and 12 female (30 %). Mean age: 36.89.
A hundred percent complained about pain which resulted in: social behaviour changes 30 (75 %), wakefulness 28 (70 %), amusement disturbances 20 (50 %), sexual disturbances 16 (40 %). Other minor disorders were sensory changes like disesthesia, paresthesia, or anaesthesia). Radiology diagnosis was utilised in all cases by means of RMI. Twenty two out of 40 (55 %) had involved L5-S1 space. Heigth (20 %) L4-L5, 6 (15 %) L3-L4, and 4 (10%) had more than one level involved.
Test on pain and influence on various aspects were run, which resulted in a pain index inability (IDD). Both RMI and IDD results were later compared. These patients underwent 10 sessions of O2-O3, two of which consisted of 6 mg/dose and 8 of 30 mg/dose. After the treatment, further tests were performed and new IDD results were obtained.
These results were later classified as follows: Excellent (35 %); Very Good (50 %); Good (10 %), fairly good (5%) and failure (0%). In fact, ozone-therapy results in an improvement in the disc-nerve root conflict and in the overall clinical picture. Such achievement is independent of the mechanical factor changes, which will take place only later in time (6 - 12 months).
It can be clearly observed that patients have improved their symptoms independently of the RMI changes, which occur only six month after ending the treatment. Pain resolution and its concomitant disabilities is the aim of the treatment in patients with acute disc herniation.

The problem of paradontium inflammatory diseases treatment is one of the most urgent problems in stomatology. In spite of an abundance of existing remedies and methods of treatment this problem is far from solution. It was noted periodontal pocket microflora accustomization to antibacterial preparations and allergic reactions and side effects to them.
The importance of searching new remedies becomes evident. The use of ozonized solutions for complex treatment of inflammatory diseases of paradontium is of great interest.
For microbiological experiment a single irrigation of pathological periodontal pockets with ozonized solutions (water, olive oil) was performed. The pocket contents before and after irrigation were inoculated on a blood agar. Microscopic specimens were prepared simultaneously. In 18-20 hours the grown colonies were counted.
After comparison of the obtained results a substantial antibacterial activity of ozonized solutions was revealed. Number of colonies before irrigation was 48.65.1 and after irrigation it was 6.2 0.3. the specimens show a significant decrease and qualitative changes of pathological pockets contents.

Prolapsus of internal genitals gives up to 15 % among indications to surgical treatment of women in gynecologic hospitals of Ukraine. The tactic peculiarity in management of patients in the postoperative period is prevention of inflammatory complications as the rate of such complications makes up to 25 %.
Usually various antiseptics are applied for sanitation of vaginal postoperative wounds, but for today such preparations are not so effective, causing allergic reactions and irritations, and do not influence local immunity of a wound.
The aim of the work is to study the effect of the ozonized olive oil on local immunity of a vaginal postoperative wound in women in whom extirpation of the uterus through the vagina have been performed on the basis of laboratory investigations.
In order to study reparative processes in the vaginal postoperative wound and a condition of local immunity we took discharge from the vagina before surgery, then wound secretion was taken from the vagina on ??, , V, V day after surgery for determination the amount of secretory immunoglobulin A (SIgA), as well as washouts from the vagina before surgery and on the 7th day after surgery - for studying phagocytosis activity and a level of immunoglobulins A, M, G by a method of radial immunodiffusion by Manchini et.al. [1].
Depending on the method of treatment of the vaginal postoperative wound the patients have been divided into 2 groups.
The first, basic group, consisted of 30 patients who were made applications of the ozonized olive oil to the vaginal wound prepared by barbotage of the olive oil with ozone-oxygen mixture with ozone concentration of 50 mg/l for 60 min.
The second, control group, comprised 30 women who have been made sanitation of the vaginal postoperative wound by usual antiseptics.
While investigating a condition of vaginal local immunity of women before surgical intervention, we found out that the amount of immunoglobulins, a level of phagocytic number, phagocytic index, and values of Ig G, A, M did not differ in washouts from the vagina in both groups.
In the basic group of women contents of Ig A became by 18 %, Ig by 9 % higher than in the control group on the 7th day after surgery with simultaneous reduction of IgM by 33 %. Phagocytic index of neutrophils in wound exudate was considerably higher in the basic group, against the background of application of the ozonized olive oil by 57 % in comparison with the control group, and phagocytic number was higher by 51 % accordingly Before surgical treatment the values of secretory immunoglobulin A did not differ essentially in both groups.
Against the background of application of the ozonized olive oil to the vaginal wound the amount of SIgA after operation increased by 20 % in comparison with the control group on the 1st day, on the 3rd day - by13 %, on the 5th day - by22 % and on the 7th day - by 15 % ??onclusions.
1. We received confirmation of the positive effect of the ozonized olive oil on local immunity that had caused additional migration of immunocompetent cells in the postoperative wound.
2. Increased level of secretory immunoglobulin A in wound secretion which positively affected the course of the wound process confirms immunomodulating effect of the ozonized olive oil. Application of the given medical complex also had the expressed action on dynamics of phagocytic number and phagocytic index.
Thus, advantage of the method of treatment of vaginal postoperative wounds with the ozonized olive oil before treatment by traditional antiseptics consists in acceleration of reparative processes in the wound and immunostimulating effect. It allowed to recommend it for application in treatment of postoperative wounds in gynecologic departments.

As it is known, ptosis and prolapsus of the internal genitals make up to 28 % in structure of gynecologic diseases. The basic method of treatment of this disease is surgery. But microbic contamination of the operative wound occurs during surgical intervention, and the rate of postoperative inflammatory complications has no tendency to reduction and makes from 7 up to 25 %.
The basic method of prevention of such complications is sanitation of postoperative wounds by antiseptics, but researches of efficacy of their application have shown their insufficient effect We have developed and introduced into practice a method of treatment of vaginal postoperative wounds with the ozonized olive oil which was prepared by barbotage of 100 ml of oil with ozone-oxygen mixture for 120 min with ozone concentration of 50 mg/l.
We have applied the prepared ozonized oil in the form of applications to a vaginal postoperative wound daily, from the end of the operation up to discharge the patient from hospital.
The aim of the work is study of general immunity condition in patients with ptosis and prolapsus of the internal genitals who are preparing for surgery and changes in the postoperative period in treatment of wounds by traditional antiseptics and with application of ozone therapy.
To control efficacy of treatment womens condition of general immunity has been studied before operative treatment and on the 7th day of the postoperative period [1, 2].
The first, control group, comprised 30 women in whom sanitation of the vagina after surgery was made by antiseptics. The second, basic group was made by 30 patients in whom the ozonized olive oil was applied to the wound. The group of comparison was made by 20 women of the corresponding age without genital prolapsus.
While investigating the immune status of women with genital prolapsus before surgery we have found impairments of immune homeostasis which was determined by -lymphopenia (reduction ofD3 + 1.4 time), decrease in the level of -suppressors (CD8 +) 1.3 time, B-lymphocytes (CD22 +) 1.2 time and natural killers (CD16 +) on an average 1.4 time in comparison with healthy women.
Parameters of humoral immunity have also been lowered: serum IgG?? 1.7 times; IgA 1.6 times and IgM 1.7 times in comparison with healthy women that were evidence of formation of secondary immune insufficiency in women with genital prolapsus. Levels of circulating immune complexes, phagocytic index and phagocytic number were almost identical to those of healthy women.
On the 7th day after surgery a group of women where ozonized olive oil was applied to the vaginal wound were observed to have an increase in practically all parameters of cellular and humoral immunity: -lymphocytes (CD3 +) have increased by 14.7 %,helpers (CD4 +) by 12.0 %, suppressors (CD8 +) - by 25 %, B-lymphocytes (CD22 +) by 43.5 % and natural killers (CD16 +) by 41.7 %.
Along with growth of amount of B-lymphocytes their functional activity increased too the amount of serum IgG has grown by 6.2 %, Ig by 21.1 %. Parameters of phagocytic index and phagocytic number reliably increased as well.
Such immune response confirms the fact that stimulation of the immune response of the mucous membranes can result in stimulation of immune mechanisms of protection both in the mucous membranes and all over the organism. There were no positive dynamics observed in using the traditional methods of treatment of postoperative wounds by antiseptic preparations.
It should be noted that healing of the postoperative wounds in women of the basic group occurred by a primary intension, in the control group complications were observed in 20 %.
Hospital stay of patients of the basic group with application of the ozonized olive oil was by 2.7 days less after surgery than of the controls.
Conclusions.
On the basis of analysis of the results obtained, we have received confirmation of the positive effect of the ozonized olive oil on general immunity due to significant restoration of parameters of cellular and humoral parts of immunity.
Thus, advantage of the method of treatment of vaginal postoperative wounds by applications of the ozonized olive oil in comparison with traditional methods shows that besides acceleration of wound healing process, there is an improvement of parameters of general immunity that allowed to recommend it for application in treatment of postoperative wounds in gynecologic departments.

A bad distribution of methods of objective evaluation of physiological effect of ozone on the organism is one of the significant problems of ozone therapy. The given paper presents the preliminary data of using the method of express-measuring of venous blood saturation with oxygen as an index of completeness degree of the ozone therapy course.
We have made a conclusion being based on the literature review analysis that the most characteristic effect of ozone on the organism is its influence on the oxygen metabolism (OM).
There have been described three main mechanisms of ozone influence on OM: 1) decreased hemoglobin affinity with oxygen (Rokitansky O., 1982); 2) decreased microviscosity of the erythrocyte membrane; 3) vasodilatation by NO-syntase activation (Gustov A., Kotov S., 1992).
It is evident that these factors should firstly influence on oxygen transport in the distal parts of the arterioles, gas exchange in the tissues and cellular respiration. There is a convenient integral index of tissue oxygen intake ??? venous blood saturation with oxygen (VBS).
In contrast to arterial blood saturation (AVS), VBS is extremely sensitive to parameters of the erythrocyte membrane, constant of hemoglobin affinity with oxygen, oxygen exchange intensity in tissues and micropearmiability of the arterioles.
To measure VBS level we have constructed a device that measures the level of oxyhemoglobin (HbO) and hemoglobin (Hb) in the venous blood. VBS is calculated by the formula OH/(OH+H). The work of the device is based on the principle of two-ray spectroscopy. The device is a part of the apparatus ???BOZON-AHTMO(produced by SIV Econika) for automatization of the autohemotherapy major (AHTM) process. This device takes a patients blood automatically, ozonizes it and returns the ozonized blood to the patient.
The duration of the autohemotherapy major procedure is 10-15 min. The device display shows concentration values of Hb, HbO and VBS as well as partial oxygen pressure in the venous blood (POPV). POPV is calculated by the Hills equation for pH=7.4.
The paper presents data in the units of POPV for pictorial presentation. The partial oxygen pressure in the arterial blood?� POPA was evaluated according to the indications of the device NONIN-9500The Figure 1 presents typical examples of POPV registration of patients??? blood during the course of AHTM. The arterial blood saturation with oxygen was within 95-98% in all patients and wasnt practically changed during the ozone therapy.
According to the Hills equation this value corresponds to POPA 97 mm Hg. On the contrary, the value of VBS and POPV undergoes considerable changes. As an example we present the POPV dynamics in practically healthy elderly man (Fig.1). It shows moderate age changes of POPV. Three sessions of autohemotherapy (100 ml of blood, the ozone dose is 4 mg) practically returned POPV to the norm. The arteriovenous difference of oxygen pressure (AVOP=POPA-POPV) increased from 39 to 52 mm Hg, i.e. by 33%.
At the level of self-evaluation the patient notes sharp improvement of tolerance to physical exertions and life quality. The second patient with a diagnosis of hereditary peroneal amyotrophy of Charcot-Marie noted episodic refusals in setting leg muscles in motion before treatment that were accompanied by falling down, pain and paresthesia in the legs as well as distal hyposthesias.
It is seen in the Figure that the disease is characterized by sharp reduction of OM. Before treatment AVOP was 26 mm Hg. The figure shows that the course of autohemotherapy major consisting of 5 procedures caused normalization of POPV. The number of the ozone therapy procedures was determined like in the first case from the dynamics of POPV normalization.
On the whole AVOP increased to 50 mm Hg (i.e. by 92%) after the course of the ozone therapy. Normalization of POPV was accompanied by reduction and disappearance of pain in the legs and gait improvement. Episodes of ataxia were not repeated during the course of the ozone therapy.
The analysis made by the end of a month???s interval since starting treatment showed some increase of VBS. In future it is intended to manage the patient by the course of peroral intake of the ozonized distillate with two month???s interval between the courses. Each course consists of 5 intakes of 200 ml of the ozonized distillate with a concentration of 15 mg/l.
Such application proved to be correct while using it in one of the patients relative suffering from the same hereditary disease. The follow-up period was 2 years. The patients condition was satisfactory during this period.
On the whole we consider that the application of the devised measuring device for VBS may improve considerably the planning of the ozone therapy course and optimize the course as to the periodicity and number of the ozone therapeutic procedures.

Ozone therapy is not officially allowed in many countries, but medical private services are using this therapy all around the world. However, appropriate control systems to assess the benefits and risks of systemic ozone therapy are not always used and in such cases the treatment is based on anecdotal reports.
Oxidative stress phenomenon is becoming as a highlighted biological process for ozone therapy because is deeply involved in its mechanism of action, and by the contrary ozone therapy is an efficient regulator of the oxidative stress processes.
In terms of therapeutic effects it is convenient to know the metabolic status of the organism to face new oxidative challenges before and during ozone therapy applications.
Oxidative stress is also important because it is involved as a cause or effect of many diseases. Since 1990 decade there is the necessity of developing reliable systems for measuring the oxidative stress that a human is undergoing.
In this sense we have proposed a system for oxidative stress diagnosis which can serve as a control system for systemic ozone therapy applications. The system is based on the blood measurement of 8 biomarkers (GSH, GPx, GST, SOD, CAT, DC, SRATB and HPT) and the interpretation of these values by an algorithm developed in a computer program yielding 4 new indices (total antioxidant activity, total prooxidant activity, redox index and grade of oxidative stress).
The system shows the patient redox status and estimation of the oxidative stress level, having these information??�s relevant?� implications on the dosage and therapeutic effectiveness of ozone therapy.

The aim of this study is to determine the effects of ozone as a therapy for patient with retinitis pigmentosa. A double blind, placebo controlled study was designed, in which 123 patients, ages 15 to 65 years, and suffering from typical forms of retinitis pigmentosa participated Sixty two patients were randomly assigned to the treatment group and 61 to the control group.
A complete general and ophthalmologic examination was performed after signing an informed consent Snellen visual acuity was measured (less than 2.5 lines considered non significant changes) and Goldmann kinetic visual field with 31.5 asb. background and V-4e target.
Visual field areas were digitized and measured with a computer program (VISUAL). Ozonated blood was administered to patients in the treatment group, in a daily fashion for 15 days, while patients in the control group received only blood.
Participants were blind with regards to this, throughout the duration of the study Visual acuity remained the same, 6 months after the treatment, in 74.2 % of the patients, improve in 21 % and worsen in 4.8 %. Visual field area remained the same, 6 months after the treatment, in 41.9 % of the patients, improve in 46.7 % and worsen in 11.35 %.
Significant differences were observed between groups, with lower frequencies of improvements in controls are larger proportions of worsening. Improvements were not observed in 91.9 % of patients one year after the treatment.
This tendency was first noted after 6 months from the treatment. Ozone treatment is proved of benefit in patients with RP, although its action is temporary. Repeated administration after 6 months is recommendable to maintain its action

Retinitis Pigmentosa is an hereditary disease of the retina which produces night blindness, peripheral visual field loss and narrowing of the central vision, which degenerates, after several years, in total blindness. This disease affects approximately 0.5 % of the world population and it is estimated that in Cuba suffer this disease, 3.1 per 10,000 habitants, with a great incidence in the central and west region of the country. In Cuba, it has been using ozone therapy like part of the treatment of Retinitis Pigmentosa, since November 1986, in “Salvador Allende” University Hospital.
A study of the clinical evolution of 20 patients that have been treated (between 5 and 10 years) with repetitive cycles of ozone therapy was performed. No other therapy was associated.
The ways of ozone application was autohemotherapy or rectal insufflation. A complete ophthalmologic examination was performed during this period. Snellen visual acuity, considering significant changes more than 2 lines and Goldmann kinetic visual field (V-4e target), considering significant changes increase in more than 10 grades and/or appearance of peripheric windows, were analyzed.
The best results were achieved when ozone treatment is repeated, at least, twice a year, along all these years, with improvement of 70 % in the visual field and 42 % in the visual acuity.
The rest of the patients maintained their visual field or visual acuity the same as the initial value, without worsen the disease. Any side effect was observed. The satisfactory results encourage to recommend its use in patients affected by Retinitis Pigmentosa.
